Create or Upload Forms in Qualee

There are 2 ways to create a form template in Qualee.

Upload existing documents

If you have an existing template (i.e. NDA, Contracts, Employee Information forms)

You can easily migrate and upload those documents in Qualee.

To upload a template

  • Select Templates in the left panel menu

  • Click Create and select Upload

  • Once uploaded, select a type of fillable content that you wish your consignee to accomplish. Contents can be found on the right panel menu.

    • Text Field

    • Initials

    • Checkbox

    • Card details

    • Signature

    • Date

    • Dropdown

    • Collect files

  • Next, create a signer role.

  • Click "Add roles"

  • Input "signer" in small letters without the quotation mark

  • Assign the role to the relevant contents

Once done, your template will now be stored in Qualee automatically, and you may choose to use the created form template as part of a "Journey" step or as send it as an "Action" to individual employees or a segment. You can create as many templates as you want and receive unlimited responses for any templates created via Qualee forms.

Create Shareable Forms

You may also create shareable forms with system-generated URLs or embed codes that you can copy and paste to your websites.

Please take note that this feature is only available in the business and enterprise plans of PandaDoc. Similar to templates, you can create unlimited forms, but only receive up to 250 total responses every year.

To create a shareable form:

  • Select Forms in the left panel menu

  • Click Create

    • Use existing template

    • Create a blank form

  • Next, publish your form.

  • Click Publish

  • Then select Get code

You may share it with your employees by sending the Web Link directly via Qualee Messaging or by inserting it as a hyperlink in the description of one of your Journey steps.
